organization dedicated to providing grocery relief and financial

Hospice Care and Care Teams
Hospice care is available to any person of any age with any diagnosis
who:
- Has a limited life expectancy if the illness runs its natural
course
- Is no longer undergoing curative treatment
- Has an identified caregiver available or, if able to provide
self-care at the time of referral, arranges to have a specific
caregiver when future circumstances dictate
- Accepts the Hospice care philosophy
- Has a primary physician who approves admission.
Hospice care can be provided in either the home or in facility-based
settings such as a nursing home, assisted living community, or hospital.
Physical, social, spiritual, and emotional care are provided during
the last stages of illness, during the dying process, and during
bereavement by a medically directed interdisciplinary team. The
team consists of patients, family members, medical directors, nurses,
social workers, home health aides, dietitians, chaplains, volunteers,
music therapists, and speech, physical, and occupational therapists.

Ashland County Launches Prescription Drug Discount
Card That Can Be Used by All Residents
Discount Cards Offering Average Savings
Of 20 Percent Off Retail Price
Ashland County has launched
a discount card program to help consumers cope with the high price
of prescription drugs. The county is making free prescription
drug discount cards available under a program sponsored by the
National Association of Counties (NACo) that offers average savings
of 20 percent off the retail price of commonly prescribed drugs.
The cards may be used
by all county residents, regardless of age, income, or existing
health coverage, and are accepted at 80 percent of the county's
pharmacies. A national network of more than 57,000 participating
retail pharmacies also will honor the NACo prescription discount
card.
"Ashland County
is proud to be one of many counties nationwide participating with
NACo in this program." said County Commissioner Mike Welch.
"The NACo prescription discount card offers significant savings
for the uninsured and underinsured residents of our county, and
even those fortunate to have prescription coverage can use the
card to save money on drugs that are not covered by their health
plan. Residents do not have to be Medicare beneficiaries to be
eligible for this program."
The Prescription Drug
Discount Cards will be available at the Ashland County Commissioners'
Office at 110 Cottage Street, Ashland, Ohio, and at the Ashland
County Department of Job and Family Services, located at 15 West
4th Street, Ashland, Ohio. Members of the Ministerial Association
will also have the Prescription Drug Discount Cards available,
as well as many health physicians' offices in Ashland County.

"Using the NACo
prescription discount card is easy," said County Commissioner
Mike Welch. "Simply present it at a participating pharmacy.
There is no enrollment form, no membership fee and no restrictions
or limits on frequency of use. Cardholders and their family members
may use the card any time their prescriptions are not covered
by insurance."
The program is administered
by AdvancePCS, a division of Caremark Rx, Inc. of Nashville, Tennessee.
The National Association of Counties (NACo)

The Veterans
Service Commission is a state agency dedicated to aiding Ohio
veterans during times of need. There are two distinct services
the agency provides.

to eligible veterans and family members who have a demonstrated

Assistance when dealing with the
Department
of Veterans Affairs (DVA). The Veterans Service
Commission represents and assists veterans, family members,
and survivors when they apply to the DVA for benefits, with
special emphasis on Veteran Compensation claims and Widows
benefits. |
Ohio is the only state
that provides such comprehensive support and services to its veterans. Each
of Ohio's 88 counties has a board of five appointed commissioners
who administer the program for their respective county. Funds
for operating expenses and benefits are provided through county
real estate taxes.

Who we are
The Ashland County Family
& Children First Council is a network of local service providing
organizations, government, advocates, parents and funders of services
for children and families. The FCFC holds as its purpose to improve
understanding and knowledge of county-based services. It serves
to identify service gaps and improve delivery for families seeking
assistance with their children. We have representatives from thirty
different county agencies

Programs are currently available?
* Owner
Rehabilitation (up to $30,000)
Taking
applications until June 1st.
The purpose of this program
is to rehabilitate a one-family housing unit, which are owned
and occupied by low and moderate-income households.
Eligible items for this program include: installation
and/or repairs to foundations, roofs , gutters,
electrical upgrades , plumbing ,
furnaces, hot water heaters , insulation
, windows , doors ,
siding , accessibility improvements and lead-based
paint abatement.
* Home
Repair - Emergency (up to $8,000)
The purpose of this
program is to provide home repairs to one-family housing units,
which are owned and occupied by low and moderate-income households.
Eligible items for this program include: emergency
repairs , handicapped accessibility ,
tap-ins, well and septic repairs or replacements, and lead-based
paint abatement.
Emergency Monthly
Housing Payments
The purpose of the Emergency Monthly Housing Payments (EmHp) activity
is to prevent household's inability to pay housing related costs
from becoming a crisis, such as eviction, foreclosure, or utility
disconnection. EmHp will assist lower income households experiencing
undue hardship and threatened with homelessness due to utility
disconnection and/or eviction (for mortgage/rents arrearages).
Furthermore, EmHp will assist individuals/families who find themselves
homeless to move into permanent housing. Through this activity,
assistance will be provided on behalf of the eligible household,
in the form of a grant to providers of such items or services
as rent, security deposits, mortgage payments, and utility payments.
In order to be eligible
for assistance, an applicant household must meet all the following
criteria:
- Be an "owner-occupant" or "renter-occupant" of a single-family
dwelling units (including industrialized/modular units and manufactured
units commonly referred to as "mobile homes") or become an "owner-occupant"
or "renter-occupant" of a single-family dwelling as a result
of EmHp assistance.
- Have a total household income at or below 80% of HUD Section
8 medium family income limited (based on household size).
